The Targeted Case Manager Screener is responsible for providing a wide variety of case management services as directed by the Targeted Case Management/Screening Department Supervisor. The Targeted Case Manager Screener is responsible for initially assisting clients in the provision of Targeted Case Management services, where in through they will gain access to needed financial and insurance benefits, employment, medical, social, education, assessment of functional abilities and needs, and other support services. These preliminary activities with the client include making initial contact with the client, establishing rapport with client where in a determination of the client’s TCM-Medicaid eligibility is made. Additionally the Targeted Case Manager Screener will complete the initial authorization request for the agency to assist the client, and administrative activities such as assigning/transferring the client to an in the field Targeted Case Manager, correspondence/communication with referral sources and other departments assigned within the agency. This individual will also follow-up with as- needed referrals to clients needing other as needed internal services.

POSITION’S ESSENTIAL DUTIES & RESPONSIBILITIES
The following are duties, responsibilities and leadership roles that the Targeted Case Manager Screener shall provide to the organization:
· Make initial phone contact with the client to complete screening or schedule appointment.
· Completes initial Screening/intake with the client via phone and/or telehealth methods.
· Reviews and explains consents and assists the client in comprehending what TCM Services consists of as well as completes consent documents via Foxit or obtains direct signature of consents on ecr (electronic client record) via zoom.
· Completes an evaluation of clients’ needs, symptoms, diagnosis (or potential diagnosis), in that it would meet Medicaid Targeted Case Management criteria.
· Provides Authorization Department the initial Transfer Summary to ensure TCM services are approved for the for ongoing assistance to client.
· Communicates with providers and referral sources in the community.
· Evaluates and makes determination of potential services that client could benefit from, and communicate this to ongoing assigned TCM.
· Assists with continuity of care through communication and corresponding to ongoing assigned providers within agency and outside providers.
· Completes a referral for additional Chrysalis Health services upon the client’s request.
· Documents TCM screening activities and other clinical paperwork within the specified timeframe.
· Documents daily activity in addendum as related to clients served throughout the day.
· Demonstrates knowledge of the Medicaid manual and all updated revisions.
· Completes ongoing chart audits to ensure compliance and quality assurance of services provided.
· Demonstrates knowledge of DSM-5 and enters a MH diagnosis in TCM chart.
· Demonstrates proficiency in utilizing community resources as supportive services.
· Ability to report to work as scheduled, in a timely fashion and with an attitude oriented toward service.
· Ability to understand and comply with all Chrysalis Health policies and procedures.
· Presents in a professional manner on digital appointments, on telephone and in written communication including times of extreme stress and when others are upset.
· Works effectively with culturally diverse groups and individuals experiencing an exacerbation of either psychiatric, substance use and/or medical symptoms, as well as experiencing a stressful situation.
· Works cooperatively in the work environment and takes direction from management with dignity, respect and in a productive, professional manner.
· Works cordially with others in a team setting.
· Assumes a professional, helpful style in all encounters with any as needed to be communicated with professionals (both internal and external) and represents Chrysalis Health in a positive manner 100% of the time.

· Ability to render compassionate care and an equal commitment to serve others in need of assistance.
· Reads, reviews and responds to electronic and hand written mail, memoranda and other correspondence and responds to phone inquiries timely (within 24-hours or next available business day) and in a professional manner.
· Accepts other special projects as assigned.
· Must complete 50 hours of certification training towards CBHCM credentials.
· Must sit and take certification exam within first year of employment and maintain certification bi-annually.
· Must maintain training requirements annually.
· Must successfully complete all criminal background checks annually.
· Must provide documentation of a valid Florida driver’s license and proof of insurance which must be maintained throughout tenure in the amount of at least
$20,000/$10,000/$20,000 for liability limits.
· Must maintain a safe driving record with no more than 6 points on motor vehicle report (MVR).
· Ability to utilize HRIS management system for payroll and evaluations.
· Carries out special assignments as directed at time of a hurricane or other natural disaster.

QUALIFICATIONS
Graduation from an accredited college or university with a minimum of a Bachelor’s Degree in Psychology, Social Work, Marriage and Family Therapy, Mental Health Counseling or related field with at least 1-year experience working with the specific population they are applying to work with (i.e. children with serious emotional disturbances or adults with serious mental illness) and:
· Must be State-Medicaid certified within the first 12 months of employment o provisionally State-Medicaid certified within 3 months of hire.
· Must have knowledge and must comply with state and federal statutes, rules and policies that affect the target population.
· Must have knowledge of the principles and practices of the discipline.
· Bilingual a plus
