    FLSA Status: Exempt

    Posting Salary Range: $54,106 - $68,173

    Office Location: Remote within Trillium’s Northern Region (See requirements section for included counties)

    What We’re Looking For

    Trillium Health Resources has a career opening for a Supports Intensity Scale (SIS) Evaluator to join our team. This position’s primary responsibilities include scheduling, completing, and the administration of the Supports Intensity Scale, including scoring the assessment, preparing a summary report, and completion of other related reports. In addition, the SIS Evaluator may conduct other assessments/evaluations as requested.

    On a typical day, you might:

    • Schedule and administer the Supports Intensity Scale (SIS) assessments as assigned, including reassessments and amendments.
    • Develop a written report of the results of completed assessments, including conclusions, support needs index scores, and supplemental questions assessment in a timely manner.
    • Assist with the provision of education/training as needed to promote positive outcomes of SIS initiative.
    • Ensure responsiveness to feedback/questions/concerns from individuals, families, and providers related to the SIS.
    • Collaborate with other members of the SIS team to ensure consistency between interviewers.

    Qualifications

    Required:

    • Bachelor’s degree
    • Two (2) years of post-degree progressive experience assessing and/or evaluating the support needs of individuals with Intellectual and/or other Developmental Disabilities.
    • Must be certified to administer the SIS, which may be attained after commencing employment with Trillium.
    • Must have a valid driver’s license.
    • Must reside within Trillium’s Northern Region, which includes the following counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, or Warren.
    • Must be able to travel within catchment as required.

    Preferred:

    • Degree in Special Education, Psychology, Social Work, or a closely related Human Services area.
